1.0 PERFORMANCE REVIEW – NEW EMPLOYEES

  • At least two weeks prior to the hiring date of the new employee, the Team Lead shall prepare the On-the-Job Coaching Program (FTF-MBA-F-002), for approval of the Director.
  • The Team Lead and the new employee shall have feedback sessions in frequencies defined in the On-the-Job Coaching Program (FTF-MBA-F-002). The Team Lead must accomplish the On-the-Job Coaching Program (FTF-MBA-F-002) with the concerned employee.
  • The Team Lead and the new employee shall have feedback sessions in frequencies defined in the On-the-Job Coaching Program (FTF-MBA-F-002). The Team Lead must accomplish the On-the-Job Coaching Program (FTF-MBA-F-002) with the concerned employee.
  • Two weeks before the 3rd and 5th months of the employee, the employee shall submit a fully accomplished Self-Appraisal Report (FTF-MBA-F-009) to their Team Lead.
  • The Team Lead shall accomplish the Individual Performance Scorecard Report (FTF-MBA-F-008) taking in consideration the gathered critical incidents and performance feedback of Self-Rater.
  • The Team Lead shall submit a fully accomplished On-the-Job Coaching Program (FTF-MBA-F-002) and Individual Performance Scorecard Report (FTF-MBA-F-008) for approval of the Director. They must discuss all concerns to ensure objectivity of the appraisal.
  • One week upon receipt of the On-the-Job Coaching Program (FTF-MBA-F-002) and Individual Performance Scorecard Report (FTF-MBA-F-008) of the employee, the Director shall approve/provide feedback.
  • Upon approval of the Director, the Team Lead shall discuss the appraisal with the employee using the approved Individual Performance Scorecard Report (FTF-MBA-F-008). The employee is encouraged to actively participate during the performance review.
  • Once the Individual Performance Scorecard Report (FTF-MBA-F-008) is signed by the employee, the Team Lead shall forward the fully accomplished On-the-Job Coaching Program (FTF-MBA-F-002) and Individual Performance Scorecard Report (FTF-MBA-F-008) to HRD, for record purposes.
  • If the employee did not pass the probationary period, the Team Lead shall discuss with the employee during the performance review the consequences of poor performance, then, serve the Notice of Termination of Employment to the concerned employee.
